Oklahoma has a form that appears to be simple but is deficient in many ways. Using the form without modification could result in unintended termination of life saving treatments on one hand or could require the use of feeding tubes to artificially prolong one's life for years. Oklahoma's Advanced Healthcare Directives (Living Will) provides three basic choices for your end-of-life care. The form is confusing and provides little guidance when completing it.
